Instructions:

1.     This page is used for nonresident alien individuals to submit GLACIER forms and document copies to be reviewed by UCLA Central Resource Unit.

2.   Review, sign and date all forms created from your GLACIER record (Tax Summary Report and all forms listed in the “Required Forms” section on your Tax Summary Report).

3.   Scan your signed/dated forms with your “Required Document Copies” as instructed on the GLACIER Tax Summary Report and upload them here. Note: The file format must be PDF (preferred) or JPG.
